Sharing Agreement
When you choose "YES -- I consent" in the "Do you consent to this Sharing Agreement, without restriction?" box below, you agree to share your dogs' records, without restriction or reservation, with the Doberman Diversity Project, Inc. (hereinafter "DDP"). You also consent to permit Embark Veterinary, Inc. (Embark) to directly transfer your dog's data and DNA test results to the DDP, without restriction. Carefully read the details provided below before you consent to such sharing.
If you are not the sole legal owner of this dog, all owner(s) of this dog MUST complete a Consent form before we can process this order. The Consent form can be found at www.DobermanDiversityProject.org/consent
What does my consent to share mean? It means you agree to permit the Doberman Diversity Project to collect, analyze and share (for the purpose of research and advancing the understanding of Doberman health, longevity, genetics, diversity, and breed sustainability), your dogs' test records which include, but are not limited to, DNA and health testing results, health records, cardiac records, general information records (including but not limited to call name, sex, country of birth, etc.), holter data, genomic information, demographic/regional, and other information collected by the DDP. Consent to share also means that you give your express consent, without limitation, to Embark to directly share any and all of your dog's data and records from Embark to the DDP on an ongoing basis.
Anonymized Records Do Not Prevent Identification of Your Dogs. While your dogs' raw genomic data from Embark Veterinary will be anonymized before sharing, linking that data with supporting health records will negate the anonymous nature of that data. For example, if genomic data and health records for a given dog are shared with a science team from universities across the world, cardiac records of that dog will be linked/associated with that dog's raw genomic data. As a result, that linking will identify which dog is associated with the otherwise anonymized data.
Why would the DDP share your dog's records? The purpose of the DDP is to collect and share Doberman data to promote transparency of the same, and to drive research by sharing freely with other research teams focused on better understanding genetic disease in the Doberman breed, and how it does or does not relate to health and longevity.
Once your dogs' records are shared, it will not be possible to stop continued sharing. It is important for you to realize, BEFORE YOU CONSENT, that once the DDP shares your dogs' data with other research teams, we cannot retrieve it or prevent further sharing. If the DDP has shared your dogs' records with other researchers, you will be advised of such sharing, and you can directly contact the organization with whom your dogs' records have been shared. NOTE that it is VERY unlikely that you can stop the sharing of your dog's data once the DDP has shared it. Do not consent to the DDP's sharing of your dogs' records under this Agreement unless you agree that the DDP is not responsible, in any manner, for sharing that occurs after the DDP has shared your dogs' records.
DO NOT CONSENT TO THIS SHARING AGREEMENT UNLESS YOU UNDERSTAND THAT THE DDP MAY SHARE YOUR DOGS' DATA/RECORDS WITH OTHER RESEARCHERS AND THAT ONCE, SHARED, IT IS NOT POSSIBLE TO STOP CONTINUED SHARING THAT MAY OCCUR WITHOUT YOUR FURTHER CONSENT AND/OR AGREEMENT. CONSENT TO SHARE ONLY IF YOU ARE COMFORTABLE WITH, AND AGREE TO, THE UNRESTRICTED SHARING OF YOUR DOGS' RECORDS.
You can always ask for your dogs' records to be purged from the DDP's records. You may instruct the DDP to delete/destroy all records pertaining to your dogs. Note well, however, that if your dogs' data has already been shared, the cleansing of DDP records will not stop continued sharing of your dogs' data outside of the DDP. If at any time, you want your dogs' data and records deleted from the DDP's records, contact us at [email protected] and the requested purge will be completed within 48 hours.
Your consent to this sharing is required for discounted testing through the DDP.
This Agreement covers ALL of your dogs tested through the Doberman Diversity Project.
